Case Manager
VPAS is accepting interested applicants for a full time Case Manager in VPAS’ Buena Vista, Lexington, Rockbridge (BLR) Region.
DAYS/HOURS: Monday through Friday 8:00 am – 4:00 pm

POSITION RESPONSIBILITIES: The Case Manager is the gatekeeper to VPAS’ in-home services, conducting home visits to determine eligibility for services and resources through a comprehensive person-centered assessment process. The Case Manager is responsible for providing information on a wide range of topics and services to individuals 60 years and older and their caregivers including, making referrals and connecting resources to meet stated needs, screening and determining eligibility for services, and providing support and guidance to navigate the complex health and human services system. The Case Manager receives all incoming VICAP inquiries through a centralized number, is trained and certified to provide Medicare Insurance Counseling, and assists all Medicare beneficiaries with resolving their questions, appeals, and enrollment issues.
QUALIFICATIONS:
Knowledge: Knowledge of the Buena Vista, Lexington, Rockbridge area, including neighborhoods and community resources within; deep understanding of the aging process and health and age-related needs of older adults and their caregivers.
Skills: Proficient in using Microsoft Office and web-based applications; strong oral and written communication, including interviewing, service documentation and assessment; organization and problem solving.
Ability: Ability to manage time and prioritize tasks, work independently and collaboratively with others; empathy and compassion for others; patience and flexibility; develop and maintain a highly organized schedule; and cultural competency.
Education: Bachelor's degree or equivalent experience.
Experience: Experience working with older adults and family caregivers in a human services environment.
